What is the difference between metabolic acidosis and metabolic alkalosis?


Introduction

As we know that every liquid form of the universe has three nature and at a time one will present that are acidic, basic or neutral.
These all terms will be decide on behalf of the solubility of that substance e.g if we dissolve any substance in water when they release H ions we called acid, the second is about base if we dissolve any substance in water and that release OH ion we called it is a basic and last about neutral if we dissolved any substance in water they release neither H ions nor OH ions we called it neutral.
Human blood is a buffer ( that resist a minor changes in its PH) if we add small amount of acid or base it will neutralize that and maintain its PH the normal PH of human blood is 7.35 to 7.45 it  mean it is neutral or slightly basic in nature as per Acid and Base scale.
According to that scale it has 15 digits and is divide into three category one is from 1 to 6  that is acid , second is only 7 that is neutral and the last one is from 8 to 15 that is basic.
There are main organs which are responsible for the management of these acidic or basic condition one is kidney which is responsible for the removal and retaining  of H ions and bicarbonates  and the second is lungs which is responsible removal and retaining of CO2.

In metabolic acidosis or alkalosis kidney play is a primary performer which maintain or either the cause of metabolic  acidosis and  alkalosis and lungs play as a secondary organ or an organ or compensation which help with the metabolic system to maintain the ph of blood.



Metabolic acidosis
              Metabolic acidosis is the disorder of metabolic system in which too much acid produce in the body during metabolism  that cause the decreasing the ph of blood that is called metabolic acidosis. During metabolic acidosis kidney is also responsible for becoming an acidic because kidney is responsible for the removal and retaining of H ions and bicarbonate which can’t remove H ion instead of H ions they remove  bicarbonate which cause the decreasing of PH as a result acidic condition occur in the body. During metabolic acidosis lungs will compensate with the metabolic system for that purpose they increase the rate of respiration due to this increased respiration rate more CO2 released from the body and the concentration of CO2 get reduced as a result PH of blood move toward normal position.
 Causes of Metabolic acidosis
1-       Increased acid production
                        It happen mostly in different way one is when we intake more and more  acid that cause becoming acidic.
Second is lactic acid formation where due to less oxygen concentration an aerobic metabolism occur which cause the formation  of lactic acid that cause decreasing the PH of blood.
Third is diabetic keto acidosis (DKA ) in which during diabetes body begin to use fat which cause the production of more and more ketone bodies in the body which cause the body to become acidic.



2-      Bicarbonate loss
Intestinal secretion contain more bicarbonates when diarrhea or any cause which initiate the loss of Bicarbonate  as a result the acidity of blood increased.
3-      Decreased acid excretion

Sign and symptoms of Acidosis
 
1-       Nausea
2-       Vomiting
3-       Abdominal pain        
8-       Headache
9-       Confusion
10-    Fatigue
11-    Drowsiness
13-  Deep rapid breathing
14-  Kussmaul respirations

16-  Arrhythmias
17-  Reduced cardiac contractility
 
In most severe case
1-       Shock
2-       Coma
3-       Death
Diagnosis
To find what actually the problem is either it is metabolic acidosis or metabolic  ketosis there is a diagnostic test called ABGs ( Atrial Blood Gase)
In which blood taken from the artery of an individual  where we can find the PH,PCO2 and HCO3.
In this test of the result happen such like the below mentioned value that should be considered
as normal if elevate or decreased than that will be acidosis or alkalosis.
PH         7.34 -7.45     if PH less than this value that will bw metabolic acidosis
HCO3     22 mEq/l -26 mEq/l if HCO3     less than the mentioned value it is toward metabolic acidosis
PCO2      35 mmHg -45mmHg if PCO2      value is smaller than the mentioned value that will be considered as metabolic acidosis.
Treatment of metabolic acidosis.
·       Treat the underline cause which lead toward metabolic acidosis
·       IV fluid
·       Bicarbonate if severe
 
 
Metabolic alkalosis
                             Metabolic alkalosis is the disorder of the metabolic system in which too much base produce in the body during metabolism that cause the elevation of PH of the blood that is called metabolic alkalosis. During metabolic alkalosis kidney is also responsible for becoming a basic because kidney is important for the removal and retaining of H ions and bicarbonates which remove most acid from the body instead of H ions which cause the elevation of  blood PH which cause the basicity of blood. During metabolic alkalosis respiratory system will decrease the rate of respiration which  cause the accumulation of CO2 in the blood  as a result the PH of blood sift toward its normal rate.
Causes of metabolic alkalosis
1-      Diuretic
Type of medicine which cause the frequent urination because of urination mire H ions and potassium ions excrete from the body as a result metabolic alkalosis occur.
2-      Loss of gastric acid
In this case vomiting and nasogastric suctioning cause the removal gastric secretion mainly hydrochloric acid and H ion which cause the lowering of H ions concentration as a result metabolic alkalosis occur.
3-      Excess bicarbonate intake
If a person intake excess amount of bicarbonate and the body absorbed it and kidney unable to excrete it out due to impaired its function due to this more bicarbonate in the blood the elevation of blood PH as a result metabolic alkalosis occur.

4-      Hyperaldosteronism
As we know that aldosterone   function  is to retain the amount of sodium in the body when it activate. In this case when aldosterone get activation it increase the retaintion of sodium and increase the excretion of H ions as a result the concentration of H ions decrease and the PH of blood elevate which we called metabolic alkalosis.
 
Sign and symptoms of metabolic alkalosis

1-       Dizziness
2-       Confusion
3-       Slow breathing
4-       Tetany
5-       Arrhythmias
6-       Palpitations
7-       Irritability
8-       Muscle cramps
9-       Muscle twitching

·       Dialysis
·       In most severe case ammonium chloride and hydrochloric acid prescribed through veinous.
